Weight Watcher's Log Where can I find one?

I am looking for a copy of a Weight Watchers Weekly weigh in log for a report I am writting. Where can I find one. It can be a copy of a used one or a copy of a new one. Does any one know where I can find one or are you willing to scan one and email to me? I am comparing vaerious weight loss programs and the vality of their use in lasting weight loss.ThanksPlease help me out on this.
